Physical, Concussion, and SCA Forms

The following hyperlinked forms must be completed before you can begin participating in a school sport:

1. TSSAA PHYSICAL - Must be dated after April 15, 2022, and signed by your physician.
2. SUDDEN CARDIAC ARREST FORM Parent and player must sign
3. CONCUSSION PROTOCOL FORM Parent and player must sign

Please turn in your completed forms to your coach in order to be cleared to play.
